The story unfolds in Yharnam, a decaying Victorian Gothic city infamous for its mysterious medical practice known as Blood Ministration, pioneered by the powerful Healing Church. This unique blood is said to cure nearly any affliction—from minor wounds to deadly diseases—drawing countless travelers over the years to seek salvation within its streets.
The player takes on the role of one such wanderer, though their goal differs from the rest. Instead of merely seeking healing, they pursue the enigmatic and cryptic substance known only as "Paleblood." After receiving a transfusion from a blood minister, the player’s journey into the nightmare of Yharnam begins.
It quickly becomes apparent that the city is consumed by a horrific epidemic. Most of its citizens have turned into grotesque, beast-like abominations, while the remaining survivors relentlessly hunt them down. To endure, the player must fight through Yharnam’s diseased streets, contending with deranged townsfolk and monstrous horrors alike.
Soon after, the player awakens in a surreal realm called the Hunter’s Dream a refuge and armory for Hunters. Here, they meet two central figures: Gehrman, a frail, wheelchair-bound mentor who offers cryptic guidance, and The Doll, a mysterious, living figure who aids the Hunter in growing stronger. Gehrman reveals that if the player wishes to find the cure they seek, they must hunt and slay the nightmarish creatures infesting Yharnam.
The game begins in first-person view through the eyes of a hunter undergoing a blood transfusion by an old man. The man warns that the hunter is about to embark on a strange journey in search of "pale blood," which will feel like a bad dream. After creating their character, the player awakens on an operating table in a small room, only to faint again upon seeing approaching creatures—a werewolf and pale humanoids. The hunter then leaves the room and discovers they are in Yharnam, specifically at Iosefka’s Clinic. Realizing the world is filled with dangerous monsters and mysteries, the hunter begins their long journey.
Soon after arriving in Yharnam, the player is transported to a spectral realm called the Hunter’s Dream, which serves as both a refuge and a workshop. There, they meet two entities: Gehrman, an elderly man in a wheelchair who offers guidance, and the Doll, a life-sized figure who allows the player to use Blood Echoes—gained from defeated monsters—to strengthen their body. Gehrman explains that to obtain the coveted blood, the hunter must eliminate the monsters plaguing Yharnam and stop the source of the infection. As the hunter ventures deeper into the city, they uncover its dark secrets and the origin of the plague. Long ago, the people of Yharnam began worshipping the Great Ones—cosmic beings dwelling in labyrinths beneath the city. These entities provided the healing blood for which Yharnam became famous, but it is also the source of the contamination. The Great Ones spread the plague to foster the birth of a new Great One infant.
While exploring central Yharnam, the second area, an NPC directs the hunter to Cathedral Ward to seek the Healing Church, which oversees blood treatment and may help locate the pale blood. After navigating the city and its depths, the hunter reaches the Ward and can send survivors to a nearby church for safety.
As the hunter progresses, they discover a skull within the Healing Church—guarded by Vicar Amelia—that grants a vision leading to Byrgenwerth College. There, the hunter encounters Rom, the Vacuous Spider, a cosmic creature destined to become a Great One. Upon defeating Rom, the hunter gains a new level of insight and witnesses Queen Yharnam and the arrival of the blood moon. They are then transported to the hidden city of Yahar’Gul, where the scholars of Mensis attempted to summon the Great Ones. After defeating a creature created by Mensis scholars to become a Great One, the hunter enters another spectral realm: the Nightmare of Mensis. There, they confront Mergo, an infant Great One, and its nurse. Only after defeating the nurse and causing Mergo’s death does the final phase of the game begin.
Back in the Hunter’s Dream, Gehrman offers to release the player from the nightmare and return them to the waking world. At this point, three possible endings can occur depending on the player's choices:
• "Yharnam Sunrise" Ending: Accepting Gehrman’s offer leads to the player being decapitated by his scythe and awakening in the real Yharnam at dawn. The Doll bids farewell in the Dream.
• "Honoring Wishes" Ending: Refusing Gehrman’s offer triggers a battle. After defeating him, a mysterious entity called the Moon Presence embraces the player, binding them to the Dream. The Doll is later seen pushing the player—now seated in Gehrman’s chair—toward the workshop, announcing a new hunt, signifying the player has become the new guide.
• "Childhood’s Beginning" Ending: Throughout the game, the player can find four Third Umbilical Cords, remnants of attempts by Great Ones to reproduce using humans. If the player consumes three before refusing Gehrman’s offer, they resist the Moon Presence and must fight it. Upon victory, the player transforms into an infant Great One. Though the transformation is unexplained, the trophy description suggests the hunter will lead humanity into a new childhood.
